Job summary

Elara Caring is seeking a dedicated Physical Therapist to deliver high-quality, patient-centered care in the home setting. You will evaluate patients, develop personalized treatment plans, and work with an interdisciplinary team to ensure optimal outcomes.

This role requires 1+ year in healthcare, an active PT license, a valid driver’s license, and the ability to lift up to 50 pounds. Join us to advance the home care industry and support patients in their homes.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license, or have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and move up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
