Job summary

Elara Caring is seeking a Physical Therapist to deliver high-quality care in the home setting across Indiana. You will evaluate patients, develop personalized treatment plans, and provide therapy services in patients' homes while coordinating with physicians and care teams.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
