Job summary

Good Shepherd Health Care is seeking a Home Health Physical Therapist to provide therapy according to current standards, including patient assessment, treatment, delegation, coordination, and evaluation. The role reports to the Home Health and Hospice Director and involves supervision of staff and students, along with comprehensive record documentation.

Qualified candidates will have a PT degree, state licensure, CPR, and clinical experience.

Qualifications

• Graduate of an approved school of Physical Therapy.
• Two years of clinical experience preferred.
• Must hold current state licensure and CPR certification.
• Ability to drive within the service area.

Responsibilities

• Interpret and carry out physician prescriptions; maintain good public relations.
• Assess patients and provide physical therapy treatment per current standards; coordinate and evaluate services.
• Supervise staff, students, and assistants; ensure proper documentation of patient records.
• Act as patient advocate and ensure timely and accurate charting.
