Job summary

Responsibilities

• Develop patient-specific care plans with measurable goals in consultation with patients, families and physicians.
• Reassess patients, document progress, and adjust therapy as needed.
• Document treatments in medical records and update care plans; mentor others as needed.
• Coordinate and direct home care for patients requiring physical therapy.
• Prioritize and adapt home care caseloads to patient needs.
• Perform evaluations and document functional outcomes.
• Apply and modify skilled interventions to achieve best outcomes.
