Job summary

AdventHealth in Altamonte Springs, FL is seeking a licensed Physical Therapist to develop and supervise patient-specific home health care plans, in collaboration with patients, families, and physicians.

This full-time role assesses and adjusts treatments, documents progress, and coordinates care to achieve optimal outcomes in home settings, with a focus on patient education and safety.

Qualifications

• Doctorate in Physical Therapy (preferred).
• Demonstrated knowledge of physical therapy theory, modalities, treatments, and skills.
• Basic computer skills and knowledge of Microsoft Office.
• Must read, write, and speak conversational English.

Responsibilities

• Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians.
• Reassesses patients at appropriate intervals, documents progress, and determines the effectiveness of therapy.
• Observes and records patient activities and findings, reporting any changes to the physician; instructs patients and caregivers on use of assistive devices and home programs.
• Documents treatments accurately in medical records and updates care plans; mentors and precepts staff as needed.
• Coordinates and directs care of home patients requiring physical therapy; prioritizes and adapts caseloads.
