Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for a full-time home health position serving Davidson County and surrounding areas in North Carolina.

You will visit adult and geriatric clients in their homes, provide rehabilitative care, and collaborate with a multidisciplinary team to help patients remain at home. The role requires NC PT licensure, at least 1 year of PT experience, and strong clinical and communication skills.

Qualifications

• Current North Carolina Physical Therapist license
• Minimum 1 year PT experience
• Graduation from an APTA-approved program
• Ability to work independently
• Strong interpersonal and organizational skills
• Proficiency with electronic medical records (EMR) preferred

Responsibilities

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, range of motion, gait, balance, strength).
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait training and balance work.
• Assess and revise care plans with nursing as part of the multidisciplinary team.
• Educate clients and families in rehabilitative care and activities for health, safety and independence.
• Document observations, interventions, and evaluations accurately using an EMR touch pad tablet.
