Job summary

BAYADA Home Health Care is seeking a part-time weekend Physical Therapist to perform home health visits in Davidson County and Randolph County, NC. The PT will assess adults and geriatric clients, develop care plans, and work with a multidisciplinary team to support safe, independent living at home.

Responsibilities include diagnostic testing, wound care interventions, EMR documentation, and client/family education. A North Carolina PT license and at least 1 year of PT experience are required.

Qualifications

• North Carolina PT license required.
• Minimum 1 year PT experience.
• Graduation from an APTA-approved program (PT).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance, safety).
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and revise individualized care plans with manual therapy and gait/balance training.
• Collaborate with nursing and the multidisciplinary team to adjust care plans as needed.
• Educate clients and families to promote health, safety and independent living.
• Accurately document observations and interventions using a tablet/EMR.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a part time weekend opportunity performing home health visits for our Davidson County Visits office. This position works with adult and geriatric clients on a per visit basis throughout Randolph County in North Carolina.

As a home health Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

• A current North Carolina Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT required.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:

Base Pay, depending upon qualifications: $68-70/point
