Now hiring for a Home Health Physical Therapist in the Gardner, Massachusetts area! This position is full-time for 6 months and will transition to PRN capacity afterward.

Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patients' needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops and implements treatment plans, performs interventions to improve patient well-being, and evaluates progress.

1. Evaluate the patient's functional status and needs; assist in developing the therapy plan of care with the physician.
2. Observe and report patient responses and condition changes to the supervising nurse or physician.
3. Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards.
4. Educate patients, families, and healthcare team members about the care plan, functional limitations, and progress toward goals.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in Massachusetts
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
