Twenty80 llc is seeking a Physical Therapist in Southern California to deliver high-quality, patient-centered physical therapy services directly in clients’ homes. This full-time role emphasizes clinical autonomy and collaboration within a home health care setting. Responsibilities include assessing patient needs, developing treatment plans, and providing therapeutic interventions.

Formación

• Graduation from an accredited Physical Therapy program (CAPTE-approved or equivalent).
• Active Physical Therapist license in the state of practice.
• Minimum of approximately 9 months of clinical experience preferred (new graduates considered).
• Valid CPR certification required or must be maintained.
• Ability to travel to patient homes as part of job responsibilities.
• Valid driver’s license and reliable transportation.

The Physical Therapist will be responsible for assessing patient needs, developing individualized treatment plans, and delivering rehabilitation services that support recovery, mobility improvement, and long-term functional independence. This role emphasizes autonomy, flexibility, and strong interdisciplinary collaboration within a home health care setting.

Key Responsibilities

• Conduct comprehensive patient evaluations in home settings to assess functional needs and mobility limitations
• Develop and implement individualized physical therapy treatment plans
