PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our home health team in Hempstead, TX. This role focuses on helping patients regain movement and reduce pain in the comfort of their homes.

Qualifications

• Active Texas PT license required.
• Graduate degree (DPT) from an accredited program.
• Professional liability insurance coverage in TX.
• Experience in home health or rehabilitative settings preferred.
• Excellent communication and education of patients and caregivers.
• Ability to adapt care plans to diverse patient needs.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Deliver therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and recommend fall prevention strategies.
• Provide compassionate care while promoting independence.
