Qualifications

Responsibilities

• Make visits to clients in their homes.
• Perform diagnostic tests and measurements of mobility and gait.
• Implement wound care and protective interventions as needed.
• Develop and implement individualized care plans including therapeutic exercises and gait training.
• Continually assess and revise the PT care plan with nursing and multidisciplinary team.
• Educate clients and family on rehabilitative care and activities to promote independence.
• Accurately document observations and interventions in the patient record.
