Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist to provide skilled services in patients' homes in Hull, Massachusetts. The role focuses on adult and geriatric patients, delivering high-quality 1:1 rehabilitative care within a multidisciplinary team.

Responsibilities include home visits, PT evaluations, creating individualized plans, and educating patients and families to foster independence. Valid MA PT license and willingness to travel required.

Qualifications

• Current Massachusetts Physical Therapist license.
• Minimum 1 year of recent clinical experience as a licensed PT.
• Ability to travel to cases as assigned.

Responsibilities

• Make visits to clients in their homes.
• Perform diagnostic tests and measurements (mobility, gait, balance, strength).
• Develop and implement individualized PT care plans including manual therapy and gait training.
• Revise care plans in collaboration with nursing and the multidisciplinary team.
• Educate clients and families on rehabilitative care to promote independence.
• Accurately document observations and care provided using tablet.
