Job summary

Traditional Home Health & Hospice in Northeast Pennsylvania is seeking a Physical Therapist (PT) to provide home-based therapy in Scranton and surrounding areas. The role offers full-time or part-time schedules with weekday hours, visiting patients in their homes and coordinating with physicians and care teams.

You will evaluate patients, develop plans of care, implement home exercise programs, and document progress in the EMR. Travel is required and independent work is common.

Qualifications

• Must possess a current, valid Pennsylvania PT license.
• Must have CPR certification.
• Valid driver's license and ability to travel to case assignments.
• Comfortable working in home settings.
• Ability to work independently with minimal supervision and manage time effectively.

Responsibilities

• Evaluate patients and develop plans of care to achieve desired outcomes.
• Provide therapeutic treatment modalities and implement home therapy/exercise programs.
• Provide instruction to patients to achieve goals.
• Maintain communication with ordering physicians and clinical team.
• Participate in quality improvement activities as assigned.
• Timely and accurate documentation (EMR) of assessments and interactions.
