Job summary

Jefferson Health at Home by BAYADA is seeking a compassionate Home Health Physical Therapist (PT) to serve patients in Lower Bucks County, PA. You will perform home visits, conduct assessments, and implement individualized care plans to help patients regain mobility and independence.

You will documenting care with a touch pad tablet and collaborate with a multidisciplinary team. A PA PT license and willingness to travel are required; enhanced pay and quarterly retention bonuses are offered.

Qualifications

Active and current Physical Therapist license in PA (PA PT).
Ability to travel to cases as assigned.
Strong communication and assessment skills; compassionate, reliable.
Calm, reassuring presence with excellent written and verbal communication.

Responsibilities

Make home visits to clients in Lower Bucks County, PA.
Perform diagnostic tests and measurements (mobility, strength, balance, safety).
Develop and implement individualized care plans including PT interventions.
Educate clients and families on rehabilitative care and independence.
Document observations and evaluations accurately using electronic records.
