Job summary

BAYADA Home Health Care in partnership with Jefferson Health at Home is seeking a compassionate Home Health Physical Therapist to serve patients in the Lower Bucks County, PA area. You will perform home visits, assess gait, balance, strength, and safety, and develop individualized care plans to promote independence.

As a PT, you’ll document care on a tablet, educate families, and collaborate with a multidisciplinary team.

Qualifications

• Active PA Physical Therapist license required.
• Home health experience welcome but not required.
• Ability to travel to cases as assigned.
• Strong written and verbal communication and assessment skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, balance, gait, and strength.
• Develop and implement individualized care plans with therapeutic exercises.
• Document observations and interventions using a tablet.
• Educate clients and family members on rehabilitative care and safety.
• Collaborate with nursing and other team members in care planning.

We are currently recruiting for a compassionate Home Health Physical Therapist (PT) to reliably serve patients throughout the Lower Bucks County, PA area. This is a high-priority area, and we’re offering enhanced pay rates plus a quarterly retention bonus for Physical Therapists interested in supporting clients in these communities.

What you’ll do:

As a Jefferson Health at Home by BAYADA Physical Therapist, you’ll use your clinical skills and compassionate heart to treat patients where they want to be the most – in the comfort of their own home. As a Jefferson Health at Home by BAYADA PT you will:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Compensation:
Base Pay, depending upon qualifications, status and subject to negotiation:

$65.00 - $70.00 per point (101,400 - $109,200 annual earning potential)

Territory Differential for Lower Bucks County - $70.00 - $73.00 per point ($109,200 - $113,880 annual earning potential) plus quarterly retention bonus payout. Must meet productivity goals to receive retention bonus.

What you should have:

• Active and current license to practice as a professional Physical Therapist (PT) in the state of PA.
• Previous home healthcare experience welcomed, but not required.
• Ability to travel to cases as assigned
• A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
• The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability.
