Job summary

Diamond Care Group is expanding and seeks an experienced Occupational Therapist to provide high-quality home health services aimed at improving patient independence and safety in their home.

You will conduct in-home evaluations, develop and implement treatment plans, assist with ADLs, train patients on adaptive equipment and home safety, complete OASIS documentation, and collaborate with nurses and other clinicians.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active Florida OT license.
• Home health experience preferred.
• OASIS experience strongly preferred.
• Strong documentation and time management skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home evaluations and functional assessments.
• Develop and implement individualized treatment plans.
• Assist patients with ADLs (bathing, dressing, grooming, toileting).
• Train patients on adaptive equipment and home safety modifications.
• Complete OASIS documentation as required.
• Collaborate with nurses, PTs, and other team members.
