Job summary

Frontier Home Health + Hospice is seeking a licensed Physical Therapist for home health services in the Fort Worth area. You will assess patients, develop individualized care plans, educate families, and document progress, coordinating with physicians and the care team.

Ideal candidates hold a CAPTE-accredited PT degree, have at least two years of relevant experience, possess strong communication and organizational skills, and maintain CPR certification along with a valid driver’s license and

Qualifications

• Must hold a valid PT license in the state.
• CAPTE-accredited degree in physical therapy.
• At least two years of PT experience, preferably in community/home health.
• Excellent verbal and written communication and organizational skills.
• CPR certification required and current.
• Must have a valid driver’s license and insured vehicle.

Responsibilities

• Conduct interdisciplinary assessment and develop individualized care plans with patients and caregivers.
• Provide counseling to patients, caregivers, and families.
• Educate patients and caregivers about treatment plans and exercises.
• Prepare clinical notes and progress summaries documenting goal attainment.
• Communicate with the physician and other healthcare providers regarding the home health plan of care.
• Provide PT services according to the physician’s plan of care.
