Job summary

Amenity Health Services 1 in Fort Worth, TX is seeking a licensed Physical Therapist to deliver patient-centered care under physician orders.

You will evaluate and treat patients, develop and adjust plans of care, coordinate with the health care team, and communicate with families to promote health and rehabilitation. A Master’s or Doctoral degree in Physical Therapy and pediatric care experience are required.

Qualifications

• Master’s or Doctoral degree in Physical Therapy.
• Experience with pediatric patients and/or developmental disabilities.
• Great customer service skills.

Responsibilities

• Provide Physical Therapy services as prescribed by the patient's physician.
• Maintain a full-time caseload of at least 30 visits weekly.
• Provide a written evaluation defining Physical Therapy needs, goals, and expected outcomes in conjunction with referring physician’s plan of treatment.
• Follow policies and procedures and adhere to all applicable patient care standards, licensure regulations, and state laws.
• Develop a written plan of care with reasonable goals and outcomes.
• Maintain and adjust the plan of care as needed for assigned patients according to identified goals and outcomes.
• Maintain patient and company confidentiality.
• Maintain effective communication between staff, healthcare team members, and family.
• Participate in coordination of services (including discharge planning) with health care team, physicians, patient/guardians, nurses and others.

Salary Range: $60.00 - $72.00 Hourly

Overview

Physical Therapist role responsible for delivering quality patient care according to physician orders. Responsibilities include evaluations, treatments, care planning, coordination, health promotion, and family training.

Key Responsibilities

• Provide Physical Therapy services as prescribed by the patient's physician
