Job summary

Concierge Home Care is seeking a Home Health Physical Therapist (PT) to provide in-home therapy with flexible scheduling across Orange Park, Clay County, FL. The role emphasizes 1:1 patient care, autonomy, and strong clinician-patient relationships.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Physical Therapist Assistants (PTAs).
• Complete documentation using streamlined technology.
