Job summary

Adoration Health in Jasper, AL is seeking a Home Health Physical Therapist to provide patient-centered care in a full-time role, Monday–Friday, 8:00am–5:00pm. You will evaluate and treat patients per physician orders, establish goals, and coordinate with families and the care team in a home health setting.

Requirements include CAPTE-approved PT education, current state licensure, CPR certification, and at least two years of adult PT experience (home health preferred).

Qualifications

• Graduate of a CAPTE-approved Physical Therapist education program.
• Current Physical Therapist licensure in State and CPR certification required; CPR may be offered upon hire
• Two years of experience in adult physical therapy; home health experience preferred
• Excellent observation, verbal and written communication skills, problem solving, basic math; PT skills per competency checklist

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Establish measurable goals and develop treatment plans toward independence.
• Maintain communication among treatment team, physician, patient and family as applicable.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT aides in direct patient care as per state practice act
