Adoration Health seeks a dedicated Home Health Physical Therapist to join our team in Moulton, AL. You will provide patient-centered care in home settings, evaluate and treat patients, and collaborate with families and physicians to improve function.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• New physical therapy graduates may be considered in select markets.
• At least 2 years of experience as a physical therapist; hospice/community experience preferred.
• Valid license to practice in the state; CPR certification current.

Responsibilities

• Perform physical therapy evaluation and treatment per physician orders, including adaptive equipment recommendations.
• Establish measurable goals and develop plans to maximize independence.
• Maintain communication among the treatment team, patient, and family.
• Maintain thorough documentation for proper billing.
• Supervise PTAs and PT Aides in direct patient care per state practice act.
