Apex Rehabilitation is seeking a Home Care Physical Therapist to join our compassionate team, providing high-quality care to clients in their homes across Long Island.

You will assess, design, and implement individualized therapy plans, guide families, and document progress, with a focus on mobility, pain management, and safe home exercises. Full-time or part-time hours with flexible scheduling; state licensure required.

Qualifications

• Degree in Physical Therapy from an accredited program.
• State licensure as a Physical Therapist.
• Strong clinical and interpersonal skills.
• Ability to adapt therapy plans to meet home care needs.
• Compassionate and patient-centered approach to care.

Responsibilities

• Conduct comprehensive assessments of clients in home care.
• Develop personalized and goal-oriented therapy plans.
• Administer therapeutic exercises and manual therapy.
• Collaborate with clients, families, and other healthcare professionals.
• Educate clients on home-based exercises and self-care.
• Maintain accurate documentation of assessments, interventions, and progress.
