Job summary

Barrette Workforce Solutions, LLC is seeking a dedicated Physical Therapist to join our home health team in Lancaster. This full-time position focuses on delivering patient-centered care in clients' homes, evaluating and treating mobility and functional goals.

You will document treatment per physician orders, educate patients and families, and collaborate with physicians and the care team to adjust plans. A CAPTE-accredited degree and active state license are required.

Qualifications

• Master’s degree in physical therapy from a CAPTE-accredited program.
• Currently licensed to practice as a physical therapist in the applicable state.
• CPR certification required.
• Valid driver’s license with ability to travel to multiple locations.

Responsibilities

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects appropriate diagnostic material.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with measurable goals.
• Educates and counsels patients to support adherence and outcomes.
• Presents patient information to physicians and informs the team of status.
• Delivers treatment within agency and payer guidelines.
• Sets objective, measurable goals collaboratively with patients and team.
• Meets productivity standards and uses work hours efficiently.
