Qualifications

• Graduate of an accredited physical therapy program with a DPT or MPT degree.
• Valid state licensure as a Physical Therapist, with current CPR certification.
• Prior experience in home health, geriatrics, or rehabilitative care preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent interpersonal and communication skills.
• Compassionate, patient-centered approach to care.
• Ability to work independently and manage time effectively.
• Proficiency in documentation software and EHR systems.
• Willingness to travel to patients’ homes and maintain a valid driver’s license.
• Commitment to upholding ethical standards.

Responsibilities

• Conduct comprehensive assessments of patients’ physical capabilities.
• Develop and implement evidence-based physical therapy interventions.
• Educate patients and their caregivers on therapeutic exercises and preventative strategies.
• Monitor patients’ progress and modify treatment plans as necessary.
• Maintain accurate documentation of evaluations, treatment sessions, and progress notes.
