Job summary

Compassus in partnership with OhioHealth at Home seeks a Home Health Physical Therapist to provide patient-centered care in patients' homes across the Southwest Columbus region. This role focuses on improving mobility and independence through individualized plans and hands-on therapy.

You'll work one-on-one with patients, coordinate with families and care teams, and help patients achieve daily living goals while balancing a flexible schedule and competitive compensation and benefits.

Qualifications

• Licensed Physical Therapist in the state where practice occurs.
• Minimum of 2 years' experience in acute or rehabilitation settings preferred.
• Ability to work in patients' homes delivering direct therapy.

Responsibilities

• Provide one-on-one Home Health Physical Therapy in patients' homes.
• Assess, diagnose, and develop individualized therapy plans to restore mobility and independence.
• Collaborate with patients, families, and care teams to meet daily living goals.
