Job summary

Heritage Healthcare Services in Las Cruces, NM, seeks a licensed Physical Therapist to provide home health services, assess and treat patients, and coordinate care under the physician's Plan of Care.

You will perform OASIS assessments, develop and adjust the plan of care with patients and caregivers, supervise PT aides, and participate in QAPI and in-service trainings.

A minimum of one year of clinical experience and a CAPTE-approved education program are required.

Qualifications

• Graduated from a CAPTE-approved physical therapy program.
• Current licensure in the state(s) of practice.
• Minimum of 1 year of clinical experience.
• Reliable transportation.
• Must be at least 21 years old.

Responsibilities

• Complete comprehensive OASIS assessments.
• Ongoing interdisciplinary patient assessment.
• Develop and evaluate the plan of care with patient and caregiver(s).
• Provide services ordered by the physician per the plan of care.
• Counsel patient, caregiver, and family; educate on procedures and home programs.
• Prepare and maintain clinical notes and communicate with physicians.
