Job summary

CareerNerds, LLC seeks a passionate Physical Therapist Assistant to provide in-home care in San Francisco, CA. You will deliver skilled interventions and develop personalized treatment plans while collaborating with a multidisciplinary team to improve patients’ mobility and quality of life.

Applicants must hold a CA PTA license, CA fingerprint clearance, CPR/BLS/ACLS, TB test and current immunizations, and a valid driver’s license with auto-insurance. Join a patient-centered home-health team.

Qualifications

Current Physical Therapist Assistant license in California.
CA Fingerprint Clearance Card.
BLS/ACLS certification.
Recent TB test and Flu Shot within the past year.
Vaccination records including Covid19, Hep B, MMR, TDaP, Varicella.
Valid Driver's License with auto-insurance.

Responsibilities

Delivering skilled physical therapy interventions, including exercises and stretches, aimed at enhancing patients' mobility and function.
Creating and executing individualized treatment plans tailored to each patient's specific needs and goals.
Collaborating with a multidisciplinary team of healthcare professionals to ensure holistic and coordinated patient care.
