Qualifications

• CA PTA license is required.
• Fingerprint Clearance Card is required.
• Current BLS/ACLS certification is required.
• Recent TB test and Flu shot within the past year.
• Immunization records including Covid-19, Hep B, MMR, TDaP, Varicella.
• Valid Driver's License with auto-insurance.

Responsibilities

• Delivering skilled physical therapy interventions in patients' homes.
• Creating and executing individualized treatment plans.
• Collaborating with a multidisciplinary healthcare team to ensure coordinated care.
