Position Overview:

We are seeking a reliable and professional Physical Therapy Assistant (PTA) proficient in providing therapy in patients' homes. We value professionals who understand the dynamics of Home Health care.

Qualifications:

• Graduate from a two-year college-level program approved by the American Physical Therapy Association
• Currently licensed in the state of Texas
• Two years of experience in Home Health preferred

Responsibilities:

• Understand and adhere to established policies and procedures
• Provide physician-prescribed physical therapy under a plan of care established by a PT
• Improve or minimize residual physical disabilities of patients
• Return individuals to their optimum and productive level within their capabilities
• Perform all skilled procedures as ordered by a physician and according to the plan of care established by a PT
• Participate with all other healthcare personnel in patient care planning
• Take initial history and perform initial evaluation
• Consult with PT regarding changes in treatment
• Instruct patients and families/significant others in home programs and activities of daily living
• Participate in mandatory in-service programs and present in-service programs as assigned
• Participate in performance improvement activities as assigned
• Attend all scheduled patient care conferences
• Prepare medical records, progress notes, and update care plans for each patient visit in a timely manner as per agency standards
