Remedy Home Health Care is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team in South Orange County. You will deliver one-on-one in-home therapy, focusing on rehabilitation, fall prevention and functional mobility.

We offer flexible per diem, part-time or full-time schedules, with opportunities for growth, supportive colleagues and a mission-driven care approach. California PTA license and CPR are required.

Qualifications

• Active California PTA license.
• CPR certification (AHA required).
• Minimum 1 year of clinical experience (home health experience preferred).
• Strong interest in home health, rehab and senior care.
• Excellent communication and organizational skills.
• Valid driver’s license, auto insurance and reliable transportation.

Responsabilités

• Provide in-home physical therapy services following the established Plan of Care.
• Assist patients with therapeutic exercises, mobility training and rehabilitation programs.
• Support senior care patients in improving strength, balance and independence.
• Monitor, document and report patient progress to the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs, safety and fall prevention.
• Collaborate with physicians, RNs and the interdisciplinary care team.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

• Provide in-home physical therapy services following the established Plan of Care
