Graham Healthcare Group is seeking a Home Health Physical Therapist to provide in-home therapy in Middleburg, Pennsylvania. The role covers a 40‑hour work week with an 8‑hour shift, delivering comprehensive patient care within a team-based environment.

The position offers a base salary range of $80,000–$120,000 per year, plus a $10,000 first-year success bonus and a $10,000 sign-on bonus. Strong EMR and device experience are preferred, with about 20% travel to patient locations.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy is required.
• Licensed Physical Therapist by the state where you work is required.
• Minimum of one year healthcare experience preferred.
• Must have a valid driver’s license and be able to drive to patient locations (~20% of day).
• Ability to meet charting deadlines and manage your schedule.

We are looking for compassionate physical therapists with:

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• A commitment to consistently meet critical deadlines for charting
• The skills needed to self‑manage your time and schedule
• Demonstrated experience with tablets, mobile phones and EMR software
