Job summary

Seaport Scripps Home Health is seeking a licensed Physical Therapist to provide in-home therapy across the South Coastal Territory in California. The PT will help patients regain mobility and independence under a physician’s plan.

The role requires an active CA PT license, at least 2 years of home health experience, strong communication, CPR certification, and a valid driver’s license with insured vehicle. Competitive compensation and comprehensive benefits offered.

Qualifications

• Active license required (California Physical Therapy License).
• Minimum 2 years of physical therapy experience (home health preferred).
• Strong communication and organizational skills.
• Current CPR certification.
• Valid driver’s license and insured, reliable vehicle.

Responsibilities

• Evaluate patients’ functional status and develop care plans.
• Assess home environments and recommend modifications.
• Provide direct physical therapy treatments.
• Educate patients and caregivers on therapeutic techniques and equipment.
• Monitor progress and adjust treatment plans.
• Collaborate with physicians and interdisciplinary teams.
• Document clinical notes and progress reports.
• Participate in discharge planning.
• Supervise Physical Therapy Assistants per policy and regulations.
