Qualifications

• Bachelor’s or Master’s in Physical Therapy from an accredited program.
• Licensed Physical Therapist in the state of Illinois.
• Minimum of one year healthcare experience.
• Authorized to work in the United States.

Responsibilities

• Collaborate with a team to provide coordinated care.
• Travel to patient locations, approximately 20% of the day.
