Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time opportunity providing home health visits in the Woodbridge, VA area, serving adult and geriatric clients.

As a home care Physical Therapist, you will be a key member of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities:

• Make home visits to clients in designated areas.
• Perform diagnostic tests such as mobility, strength, balance, and safety assessments.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and revise individualized care plans, including manual therapy, gait training, and balance exercises.
• Assess and update care plans regularly, collaborating with nursing staff.
• Educate clients and families on rehabilitative activities to promote health and independence.
• Document observations and interventions accurately using electronic devices.

Qualifications:

• Current VA Physical Therapist license.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time efficiently.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
