Elite Care Management, Inc. is seeking a Home Health Physical Therapist to deliver skilled PT services in patients' homes across Lake County, Indiana. This part-time/PRN role offers per-visit pay and flexible scheduling, with travel reimbursement and weekly pay.

You will evaluate patients, design individualized treatment plans, and implement interventions to improve mobility, balance, safety, and independence. Strong communication, documentation, and EMR proficiency are essential.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Indiana Physical Therapist license.
• Previous home health experience required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to travel throughout the service area.
• Proficiency with electronic medical records.

Responsibilities

• Conduct in-home PT evaluations and reassessments.
• Develop and implement individualized home treatment plans.
• Provide interventions: strengthening, gait, balance, transfers, mobility.
• Educate patients/caregivers on home exercise programs.
• Promote fall prevention and home safety practices.
• Monitor progress and adjust plans as needed.
• Coordinate with physicians, nurses, and therapy staff.
• Assist in discharge planning and referrals.

Position Overview

This is a part-time/PRN, per-visit position offering flexible scheduling.

The Home Health Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, and providing skilled therapy services to improve mobility, strength, balance, safety, and functional independence within the home environment.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations and reassessments in patients’ homes.
• Develop and implement individualized treatment plans based on physician orders and patient goals.
• Provide skilled therapeutic interventions, including strengthening, gait training, balance activities, transfer training, and functional mobility exercises.
• Establish and educate patients and caregivers on individualized home exercise programs.
• Provide education on fall prevention, home safety, proper body mechanics, and assistive device use.
• Monitor patient progress, evaluate treatment effectiveness, and modify treatment plans as clinically indicated.
• Communicate changes in patient condition and coordinate care with physicians, nurses, therapy staff, and other interdisciplinary team members.
• Participate in care coordination, discharge planning, and appropriate patient referrals.
• Maintain compliance with agency policies, professional standards, and applicable state and federal home health regulations.
