Job summary

Elite Care Management, Inc. is seeking a Home Health Physical Therapist (PT) to provide skilled therapy in patients' homes throughout Lake County, Indiana. This part-time/PRN, per-visit role emphasizes flexible scheduling and hands-on patient care.

The PT will evaluate patients, develop individualized plans, and deliver interventions to improve mobility, balance, safety, and functional independence. Travel reimbursement and weekly pay support a autonomous practice.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, active Physical Therapist license in the State of Indiana.
• Previous home health experience is required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to independently manage patient visits and scheduling.
• Reliable transportation, valid driver’s license, and ability to travel throughout the assigned service area.
• Proficiency with electronic medical records and mobile documentation.
• Ability to meet the physical demands of providing therapy services in patients’ homes.

Responsibilities

• Conduct comprehensive physical therapy evaluations and reassessments in patients’ homes.
• Develop and implement individualized treatment plans based on physician orders and patient goals.
• Provide skilled therapeutic interventions, including strengthening, gait training, balance activities, transfer training, and functional mobility exercises.
• Establish and educate patients and caregivers on individualized home exercise programs.
• Provide education on fall prevention, home safety, proper body mechanics, and assistive device use.
• Monitor patient progress, evaluate treatment effectiveness, and modify treatment plans as clinically indicated.
• Communicate changes in patient condition and coordinate care with physicians, nurses, therapy staff, and other interdisciplinary team members.
• Participate in care coordination, discharge planning, and appropriate patient referrals.
• Maintain compliance with agency policies, professional standards, and applicable state and federal home health regulations.
