Job summary

Elite Care Management is seeking a dedicated Home Health Physical Therapist (PT) to provide skilled therapy in patients' homes across Lake County, Indiana. This part-time/PRN, per-visit role offers flexible scheduling and competitive per-visit pay.

The PT will evaluate patients, develop individualized plans, and provide interventions to improve mobility, safety, and independence within the home setting. Travel reimbursement, weekly pay, and comprehensive support are provided.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, active Physical Therapist license in Indiana.
• Home health experience is required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to independently manage patient visits and scheduling.
• Reliable transportation and ability to travel throughout the service area.
• Proficiency with electronic medical records and mobile documentation.
• Ability to meet the physical demands of home-based therapy.

Responsibilities

• Conduct comprehensive physical therapy evaluations and reassessments in patients’ homes.
• Develop and implement individualized treatment plans.
• Provide skilled interventions to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on home exercise programs.
• Educate on fall prevention and home safety.
• Monitor progress and modify plans as needed.

Position Overview

This is a part-time/PRN, per-visit position offering flexible scheduling.

The Home Health Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, and providing skilled therapy services to improve mobility, strength, balance, safety, and functional independence within the home environment.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations and reassessments in patients’ homes.
• Develop and implement individualized treatment plans based on physician orders and patient goals.
• Provide skilled therapeutic interventions, including strengthening, gait training, balance activities, transfer training, and functional mobility exercises.
• Establish and educate patients and caregivers on individualized home exercise programs.
• Provide education on fall prevention, home safety, proper body mechanics, and assistive device use.
• Monitor patient progress, evaluate treatment effectiveness, and modify treatment plans as clinically indicated.
• Communicate changes in patient condition and coordinate care with physicians, nurses, therapy staff, and other interdisciplinary team members.
• Participate in care coordination, discharge planning, and appropriate patient referrals.
• Maintain compliance with agency policies, professional standards, and applicable state and federal home health regulations.
