Inspired Homecare is seeking a part-time Home Health Physical Therapist to join our team serving Willow Springs and surrounding neighborhoods in Illinois. Work-life balance with the freedom to schedule visits while making a meaningful impact on patients in their homes.

You will perform home-based assessments, develop individualized treatment plans, and provide high-quality care. Collaboration with a multidisciplinary team is essential to coordinate care and support patient goals.

Qualifications

• Degree in Physical Therapy from an accredited university/program.
• Proven ability to perform clinical assessments and document outcomes.
• Ability to follow policies and procedures and ensure compliance.
• Strong organizational and communication skills.
• Experience with Home Health and OASIS documentation preferred.

What matters most

Here's what we're looking for:

1. A degree in Physical Therapy from an accredited university/program.

2. A current, unrestricted state license as a Physical Therapist in the state of practice.

3. Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.

4. Strong organizational and communication skills.

5. Experience in Home Health Therapy and completion of OASIS documentation is preferred.
