Full-Time or Part-Time Physical Therapist - Home Health

Flexible Schedule, Work/Life Balance, and Great Benefits Package

BAYADA Home Health Care is currently seeking an experienced Full-Time or Part-Time Physical Therapist to provide exceptional rehabilitative care to adult and geriatric patients in their homes in Sturbridge, Southbridge, Charlton, Dudley and surrounding towns. Prior home care experience is not required. As a home health physical therapist, you will be a key member of a multidisciplinary healthcare team providing skilled nursing and rehabilitative care, helping clients maintain their independence. You may be eligible for a $5,000.00 Sign-On Bonus!!!

Responsibilities include:

• Making visits to clients in their homes
• Performing diagnostic tests and measurements, such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments
• Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions
• Assessing and revising the care plan and collaborating with nursing staff as part of the multidisciplinary team
• Educating clients and family members on rehabilitative care and activities to promote health, safety, and independence
• Documenting observations, interventions, and evaluations accurately using a tablet device

Qualifications include:

• A current Physical Therapist license in Massachusetts
