Job summary

Elara Caring is seeking a Home Health Physical Therapist (PT) to serve Harker Heights and surrounding areas. This full-time role involves visiting patients in their homes Monday through Friday, 8a-5p, within the Harker Heights Territory.

You will deliver skilled PT services in patients’ homes, develop personalized treatment plans, and collaborate with physicians and an interdisciplinary team to drive meaningful outcomes.

Qualifications

• Graduation from an APTA-approved physical therapy program.
• Current, unrestricted Texas PT license.
• At least one year of experience in a healthcare facility.

Responsibilities

• Evaluating functional needs and creating individualized treatment plans.
• Providing skilled physical therapy in the home.
• Tracking progress and adjusting care as needs change.
• Teaching patients and caregivers strategies to support recovery between visits.
• Coordinating care with physicians and other healthcare professionals.
• Completing timely, accurate clinical documentation.
• Supervising therapy assistants and support team members as appropriate.

What You’ll Need

• Graduation from an APTA-approved physical therapy program
• A current, unrestricted Texas PT license
• At least one year of experience in a healthcare facility
• A valid driver’s license and acceptable driving record, or reliable transportation between patient homes and work locations
• Ability to frequently lift, push, pull, and support up to 50 pounds, including patient positioning, transfers, and moving equipment
