Job summary

Signature Healthcare at Home in Washington state is seeking a Physical Therapist to join our Snohomish County home health team, offering a $15,000 sign-on bonus and mileage reimbursement.

You will assess and implement individualized care plans, educate patients and families, supervise PT assistants, and complete daily documentation. WA licensure and a CAPTE-accredited degree (DPT or equivalent) are required, plus BLS/CPR.

Qualifications

• WA licensure to practice Physical Therapy in the state of Washington.
• Graduated from a CAPTE-accredited program with a Doctorate of Physical Therapy or equivalent.
• BLS/CPR certification from American Heart Association or American Red Cross.
• Valid driver’s license and personal vehicle for travel to patient homes.

Responsibilities

• Develop and implement an individualized plan of care in partnership with patient and family.
• Assess and evaluate therapeutic status and contribute to the total care plan.
• Provide patient, family, and caregiver education and progress summaries.
• Instruct and supervise PT assistants per policy and state regulations.
• Complete daily documentation and scheduling.

We offer the best of both worlds –The backing and stability of a proven healthcare “giant”, with the permission to operate locally as if it were our own Snohomish County based business.Schedule, Pay and BenefitsFull-time Monday through Friday, flexible scheduling.Hourly $55 - 65 /hour DOE + Mileage Reimbursement.Medical/Dental/Vision available the 1st of the month after hire.Paid company holidays, 2+ weeks PTO and 401k match after 90 days.Duties and ResponsibilitiesAssumes the responsibility for the on-going interdisciplinary and individualized plan of care in partnership with the patient, family/representative and/or caregiver(s).Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
