CenterWell Home Health seeks a licensed Physical Therapist to provide home-based therapy, improving function and daily living activities for patients. You will evaluate progress, plan rehab programs, and supervise support staff, ensuring clear communication with the care team and family.

Role includes travel to patients' homes, 40-hour work week, and collaboration with physicians and other therapists. Benefits begin day one, with tuition reimbursement opportunities and a driver program.

Formación

• Current CPR certification required.
• Experience collaborating with a team of healthcare professionals.
• Current and unrestricted Physical Therapy license.

Responsabilidades

• Plan and administer prescribed physical therapy treatment for patients in home settings to attain highest physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement rehabilitation programs in line with the physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to physician, Clinical Manager, care team, patient, and family, and submit all required documents.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

Required Qualifications

• Current and unrestricted Physical Therapy license
• Current CPR certification
• Experience collaborating with a team of healthcare professionals

Preferred Qualifications

• One year of experience as a Physical Therapist

Pay Range

• Hourly: $76.00 - $106.00 per visit/unit
• Annual: $111,600 - $153,600

Additional Information

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.

This role is part of Humana's driver safety program and therefore requires a valid state driver's license and personal vehicle liability insurance with minimum limits of $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 property damage or whichever is higher.
