CenterWell Home Health seeks a Home Health Occupational Therapist Assistant to support patients at home under the supervision of a licensed Occupational Therapist. You will implement treatment plans, assist in rehab activities, and help patients regain independence in daily tasks.

The role emphasizes home-based rehabilitation, coordination with the interdisciplinary team, and caregiver instruction to sustain progress and safety in the patient’s living environment.

Formación

• Current and unrestricted OTA licensure.
• A valid driver’s license, auto insurance, and reliable transportation.
• Six months occupational therapist assistant experience (preferred).
• Current CPR certification (preferred).
• Home Health experience (preferred).

Responsabilidades

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
• Assist in the implementation of vocational/education programs and activities established by the Occupational Therapist.
• Design/adapt equipment and working and/or living environment; fabricate devices to assist and improve function and independence.
• Provide therapeutic treatment and instruction to patients as directed by the OT and physician orders to improve/restore strength, coordination, ROM and function.
• Report information and observations to OT and/or Clinical supervisor; document in patient records; prepare progress reports; maintain required documentation.
• Instruct patients and family members regarding home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning and act as resource to other health care personnel.
• Design community reintegration activities to assist physical reconditioning and coordinate plan with interdisciplinary team.
