CenterWell Home Health, a Humana company, is seeking an Occupational Therapy Assistant to join our Home Health team in West Virginia. You will provide therapy under the supervision of an Occupational Therapist, support rehabilitation plans, and help patients regain independence in daily activities within their homes.

The role includes documenting progress, coordinating with the care team, and guiding patients and families in home-based programs.

Qualifications

Current and unrestricted OTA licensure.
Current CPR certification.
A valid driver's license, auto insurance, and reliable transportation.

Responsibilities

Provide therapy services planned, delegated and supervised by the OT in accordance with the patient's plan of treatment.
Assist in implementing vocational/education programs designed to restore, reinforce and enhance task performance.
Design/adapt equipment and living environment to promote independence.
Fabricate devices to assist function and independence where possible.
Provide therapeutic treatment and instruction to improve/restore strength, coordination and range of motion.
Document patient observations and progress in records and clinical notes.
Assist with progress reports and maintain required documentation.
Instruct patients and family on home programs and use of adaptive equipment.
Participate in care coordination and discharge planning with the interdisciplinary team.
Contribute to community reintegration activities and coordinate plans with team.
