Job summary

CARE FIRST HOME HEALTH CARE INC is seeking a Certified Occupational Therapy Assistant- Home Health to work under the supervision of a Registered Occupational Therapist. The OTA assists in implementing standards of care and carries out services planned and supervised by the OT.

Responsibilities include evaluating patient function, teaching rehab activities, maintaining accurate records, following treatment plans, coordinating with the care team and educating families.

Qualifications

• Graduate of an occupational therapy assistant curriculum.
• Licensed by the State Board.
• Must possess and maintain CPR certification.

Responsibilities

• Participates in the ongoing evaluation of patient’s functional status as delegated by the registered occupational therapist.
• Participates in teaching new skills or retraining patients in daily activities.
• Maintains clinical records and reports changes to the supervising OT.
• Submits clinical documentation in accordance with organization policy.
• Follows treatment program and documents patient’s response and progress.
• Maintains communication with other personnel involved in the patient’s care.
• Attends in-services, case conferences, and other meetings as required.
• Identifies patient/family needs for other home care services and assists with referrals.
• Instructs patient’s family/caregiver and other staff in the treatment regime as delegated.

Education
