Job summary

Humana Inc. and CenterWell Home Health seek a qualified Occupational Therapy Assistant to join our patient-focused team in Wisconsin.

You will work under an OT to deliver home-based therapy, helping patients regain strength, mobility and independence. Responsibilities include implementing treatment plans, documenting progress, coordinating with the interdisciplinary team, and educating patients and families on home programs and adaptive equipment.

Qualifications

• OTA licensure current and unrestricted.
• CPR certification current.
• Valid driver’s license, auto insurance & reliable transportation.
• Six months OTA experience preferred.
• Home Health experience preferred.

Responsibilities

• Provide therapy services planned, delegated and supervised by the Occupational Therapist in accordance with the patient's Plan of Treatment.
• Assist in implementing vocational/education programs and activities established by the OT.
• Design/adapt equipment and living environments.
• Fabricate devices to assist function and independence.
• Provide therapeutic treatment per physician orders to improve strength, ROM and function.
• Document observations in patient records and prepare clinical notes.
• Assist with progress reports and maintain required documentation.
• Instruct patients and families on home programs and adaptive equipment.
• Participate in care coordination and discharge planning with the interdisciplinary team.
• Coordinate the plan with team members for community reintegration.
