Optum, in partnership with Caretenders, is seeking a licensed Occupational Therapist for Home Health. Assess and evaluate patients\' functional status, daily living activities, and adaptive equipment needs within the plan of care.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to a vehicle or reliable transportation.

Responsibilities

• Provides services within the scope of practice as defined by state law and the plan of care.
• Evaluates patient functional status and determines therapy needs; consults with physician for care plan.
• Observes, records, and reports patient responses to treatment and changes in condition.
• Conducts patient assessments evaluating functional ability using standard tests.
• Function in diverse home environments while upholding patient dignity and equity.
