Job summary

VNA of Maryland - Chestertown, part of LHC Group within the Optum family, seeks an Occupational Therapist in Home Health. You will assess functional status, ADLs, fine motor skills, and home safety to guide therapy plans.

The role requires active OT licensure in the state and a valid driver’s license; benefits and sign-on bonus are offered. You will collaborate with a multidisciplinary team in patients' homes.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance or access to dependable vehicle.

Responsibilities

• Provides OT services within scope of practice per state laws and plan of care.
• Evaluates functional status and coordinates care with the health team.
• Observes and reports patient response to treatment and changes in condition.
• Conducts patient assessments of functional ability using standard tests.
• Ability to work in home settings regardless of client background.
