Job summary

PARS Therapy is seeking a dedicated Home Health Occupational Therapist to support patients in Wharton, TX, delivering personalized, goal-oriented care in the patients' homes. You will help clients regain independence and function through evaluation, intervention, and rehabilitation, focusing on mobility and daily living activities.

The role emphasizes collaboration with families and caregivers, documenting progress, and applying evidence-based practices in a flexible home health environment.

Qualifications

• Active Occupational Therapist license in TX is required.
• Professional liability insurance specific to TX OT practice is required.
• Graduate degree or higher in Occupational Therapy from an accredited program.
• Experience in home health or rehabilitation settings is preferred.
• Strong communication and ability to build rapport with diverse patients.

Responsibilities

• Develop and implement individualized home health OT treatment plans.
• Assist in delivering patient-centered interventions under supervision where appropriate.
• Educate patients and caregivers on adaptive strategies for ADLs/IADLs.
• Assess home environments and recommend safety modifications.
• Document care and track progress toward goals.
• Collaborate with families to support recovery in the patient’s home.
