Christus Home Care in Athens, TX is seeking a full-time Occupational Therapist to join our compassionate home health team. You will assess and address patient needs related to daily living activities, fine motor skills, and home safety, aligning with CMS guidelines and care plans.

Key responsibilities include evaluating functional status, coordinating with physicians, documenting progress, and delivering evidence-based therapy.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsabilités

• Provides OT services within the scope defined by state laws and plan of care, coordinating with the health care team.
• Evaluates patient functional status and CMS-related documentation with physician guidance.
• Observes, records, and reports patient response and changes to condition to the nurse supervisor and physician.
• Conducts assessments evaluating functional ability using appropriate tests.
